When you contact us, we will send you our new patient intake forms to complete. This will help our providers learn about your medical history. Our forms are online forms which makes them easy to complete, so there is no need to print, scan and return.
We Will Respond
TWO
A provider will review your request and respond to you within 24hours. You will also receive a request to schedule an appointment with your provider.
In case of worsening symptoms or suicidality, please call 911 or go to your nearest ER or psychiatric hospital for assessment and help.

Schedule your appointment with your provider. During your consultation with your provider, you will develop a customized treatment plan to optimize your health and meet your goals
